Grouping visits into claims

Sometimes visits need to be grouped in a certain way in the CMS 1500 for billing purposes.

Visits are grouped in a claim if they are for the same client, same payer, and same PA number (authorization number).

Additionally, if 2 or more visits are also done by the same therapist at the same place of service, those are merged into one single service line at the claim level.
